Transitional
Housing Program
Legacy of Care offers comprehensive housing solutions to individuals and families in need of stable housing and ongoing support.
Our Legacy Pathways to Permanent Housing (LPPH) program serves as a crucial step for families transitioning from homelessness to financial and housing independence. Over a two-year period, this program provides tailored support, guiding families through the challenges of securing permanent housing while addressing financial barriers.
Exclusively available to current Legacy of Care program participants, the LPPH program offers intensive assistance and encouragement, empowering families to achieve educational, career, and financial milestones necessary for breaking the cycle of homelessness and housing insecurity.
Central to the LPPH program is the development of individualized action plans, with clear milestones designed to track progress towards financial and housing autonomy. Throughout the program, families contribute a portion of their income towards rent and utility bills, fostering budgeting skills and financial responsibility under the guidance of dedicated case managers.
Upon successful completion of the two-year program, families graduate with the assurance of stable housing, equipped with the skills necessary for sustainable independent living.
Permanent
Supportive Housing
Legacy of Care provides ongoing support to individuals through our permanent supportive housing programs. This model ensures that participants receive consistent monitoring of housing stability and progress towards personal goals, offering a secure and stable living environment for those in need.
Legacy of Care initiates its housing assistance program by providing emergency shelter support to individuals in need.
Individuals seeking shelter assistance are connected with Legacy of Care directly through outreach efforts and/or community partnerships.
Upon entering the Legacy of Care program, individuals are paired with a dedicated case manager. The case manager conducts a thorough intake assessment, addresses immediate needs, and acquaints the individual with program resources. Together, they develop a tailored action plan with short- and long-term goals aimed at achieving housing independence.
Even as individuals transition into permanent housing, Legacy of Care continues to provide ongoing case management support. This support extends for a period of two years, facilitating self-sufficiency and offering guidance to prevent individuals from returning to homelessness.

Introducing Legacy Employment Readiness Program: Empowering Financial Stability and Career Growth
In our pursuit of fostering lasting change and breaking the cycle of homelessness, Legacy of Care is proud to unveil the Legacy Employment Readiness Program. Grounded in the belief that financial education and career development are pivotal in securing housing and achieving independence, this program offers comprehensive support tailored to individuals who have faced homelessness or housing instability due to financial mismanagement.
Recognizing the pressing need for economic mobility, especially in the face of Jackson’s challenging housing market, our program is designed to equip clients with the tools and guidance necessary to navigate their path towards stability. Through our Economic Mobility Services, we address the unique challenges faced by those experiencing homelessness or housing insecurity.
From the outset of their journey with us, clients collaborate with dedicated case managers to devise personalized action plans, encompassing short- and long-term SMART goals. For individuals requiring specialized assistance, such as addressing low credit scores, managing debt, or embarking on a career search, our Economic Mobility Services department steps in to provide targeted support.
